Effects of Constraint-Induced Movement Therapy and Botulinum Toxin A Promising Combination in Children with Hemiplegic Cerebral Palsy

Abstract

Objective: Modified constraint-induced movement therapy(CIMT) and chemodenervation with botulinum toxin A are each efficacious in managing stroke-induced motor disorders; however, the application of these two promising modalities in combination is yet to be examined. The purpose of this study was to determine the effect of CIMT and botulinum toxin A promising combination on the involved upper extremity function in children with hemiplegic cerebral palsy(CP). Design : A before and after trial intervention study. Participants : Five hemiplegic CP (3 t o 4 years o f age), who had been treated by general rehabilitation therapy during previous 6 months and showed no functional improvement were studied. Botulinum toxin A was injected into target muscles with electromyographic guidance and then their unaffected arms were immobilizated by scotch short arm casts for 6 weeks. And, the therapeutic effects of this method were evaluation by Modified Ashworth scale, Grading o f Unilateral Hand Skills of Reddihouh etc., Functional classification of Hand before and after CIMT of Mowery, Erhardt Developmental Prehension Assessment. Results : 1. Modified Ashworth scale : Mean MAS decreased significantly after CIMT. Before the CIMT method, hemiplegic cerebral palsies could transfer a mean of 2.20÷0.46, and after 1 month the CIMT method a means of 1.87÷0.30, and 3 month the CIMT method a means of 1.87+0.29. 2. There were an improvement in functional classification and a signification increase of mean Score of unilateral hand skill after CIMT in patients. 3. There were no adverse effect resulting from the CIMT on an unaffected side arms. Conclusion : We concluded that the botulinum toxin A and CIMT promising combination can improved the involved upper extremity function and movement in children with hemiplegic cerebral palsy.
