Abstract
Shoulder problem is one of the issues can be seen in adult hemiplegia, and these have been a lot of researches about this theme. Symptoms usually appeared are pain, subluxation and joint contracture, reflex sympathetic dystrophy. It was difficulty in regaining independence in the activities of daily life because the pain and stiffness interfere with dressing and washing, turning over in bed etc. Balance reaction are prevented both in sitting and standing, and the patient is afraid to move freely to carry out the tasks required of him. Patient's morale is drastically lowered, and like any other person with constant pain he becomes depressed. A vicious circle ensues. Pain itself can inhibit muscle activity, and it is very difficult to stimulate the return of active movement in the hemiplegic arm while pain persists. This paper outlines some of the literature relating to shoulder problem associated with hemiplegia. When treating shoulder subluxation, various supports should be evaluated to optimize the function of the affected e x t r e m i t y and the reduction of the shoulder subluxation. Use of overhead pulley has the highest risk of developing shoulder pain and should be avoided during rehabilitation of stroke patient. The successful treatment varies according to the problem, and care must therefore be taken to differentiate between them.
